ELIGIBILITY CRITERIA

  • Active and documented community service.
     

  • High School up to completed Freshman year of college.
     

  • Minimum 3.0 GPA
     

  • Recipients can be male or female.
     

  • The Josephine G. Cook Foundation is non-discriminatory and accepts applicants regardless of race, color or ethnic background.
     

  • Applicants should be exemplary in personal character, morality and leadership.
     

  • Applicants must not have been convicted of a felony crime.

APPLICATION PROCESS

  1. Complete the online application.
     

  2. Submit the following documents:
     

  • 3 letters of reference/recommendation.
     

  • A current transcript. 
     

  • A community service log.
